2. INTRODUCTION
Livity Limited ("Livity", “we”, “us”, “our”) is a creative agency that works with young people and brands to build a better future. Our website can be accessed here. We are conducting a research and co-creation project called the "Future Report" in collaboration with Google (the "Project"). This document combines both consent for your participation in research tasks and your release for content creation as detailed below.

3. WHAT IS THIS PROJECT ABOUT?
We’re looking for passionate and creative young people just like you to get involved in The Future Report, run by Livity in partnership with Google.
You’ll team up with other teens across Europe to share your thoughts on how technology impacts your life. We’ll explore how technology can be used to solve challenges and build a brighter future for everyone.
No experience is required, just an ability to problem-solve and a creative mind. The opportunity happens entirely online, is paid, and comes with a certificate of achievement from Google. Taking part will help you build crucial skills for your CV while helping to shape future digital literacy and AI education for young people.

4. WHAT HAPPENS IF I DECIDE TO TAKE PART?

Below we have set out the activities:

2 x Online focus groups - a video call with your
in-country point of contact and up to 4 other people
your age, where you’ll be asked questions and
invited to share your thoughts and opinions. Duration: 2 x 1
hour sessions

Homework” - A short series of questions which
we’ll ask you to record your answers to using voice
notes and video. This can be done on your phone or
on a laptop. Instructions will be shared with you w/c
30th April. Duration: 1 hour

Incentive: £100 for all tasks

“Research” refers to any activity where you answer questions, complete surveys, or take part in focus groups to share your thoughts and opinions as part of the project.

“Content” refers to any activity where you create videos, images, drawings, or other media as part of the project and any Livity or Google captured video, photos, or audio of you in the focus
groups or as part of the homework.

5. WHO IS INVOLVED?
There are over 10 young people who are taking part in activities in the UK. You’ll meet a few of
them in the above groups.
6. REQUIREMENTS FOR PARTICIPATION
● All participants under 18 require parental or legal guardian consent, meaning they will need to review and sign this Consent and Release Form. Parental consent is mandatory for participation in all project activities, including Research tasks, Content creation and Content submission.

7. PARTICIPANT RIGHTS AND DATA

What are my rights if I take part?
● Voluntary Participation: It is your choice to take part. You can stop at any time.
● Privacy: If you take part, we’ll record our conversation (audio) and take notes. We do not share your contact details (name/address) with anyone outside the Livity research team without your permission.
● Media Rights: By signing this form, you agree that Livity and Google may capture and use the Research and the Content in our internal and external project reports, websites, social media and in PR, press and in the media. In addition, you agree that Livity and Google may share the Research and Content with partners, NGOs, and other stakeholders on digital policy issues. The rights in the Research and the Content are granted worldwide and "in perpetuity" (forever) so we can keep a permanent record of the project’s impact.
● Privacy, Data Processing and Retention: We will keep a record of your name, bank account and contact details (your email address and phone number) so that we can contact you if necessary and pay you. In addition your first name, age, country, image and likeness may be made publicly available when the Research and Content is published on social media, websites, PR materials or other public channels. This means that Livity and Google will process personal data when using this information. We keep a file with your name and information on our encrypted drive. Anything else you share (like
photos or tasks) and our recording and notes are labelled anonymously (for example, as ‘Participant 1’). This is held securely on our server. We delete your contact details and bank account details within 1 year. Anonymised quotes and media captured for the report will be kept indefinitely as part of the project archive. Once published, the Research and Content may be accessed by audiences worldwide and may be copied or shared by third parties outside the control of Livity and Google. The lawful basis for this processing is consent under Article 6(1)(a) UK GDPR/EU GDPR.
● Withdrawal of Consent: You can withdraw your consent for us to use your data at any time by emailing [email protected]. Note: if the Research or Content have already been published in a printed report or widespread digital media, it may not be possible to remove them after publication. 8. TAX & PAYMENT INFORMATION
● Upon completion of all applicable activities for the Project, and after collecting the Participant’s payment details (or the payment details of their parent/legal guardian if below 18), Livity will pay the agreed-upon Payment as set out in Clause 4. Payments will be made within 30 days of the participant's completion of all activities, unless otherwise agreed.
● For all participants under the age of 18, payments will be made directly to the parent
or legal guardian, regardless of the participant’s age or local legislation.
● Payment will be made via bank transfer.
● Livity does not withhold or pay income tax in participant countries. Participants
above the age of 18 or, if below 18, parents/legal guardians are responsible for ensuring compliance with local tax regulations and submitting tax declarations as necessary
and by signing, the Participant or parent/legal guardian (as applicable) confirms they understand and accept responsibility for any applicable tax declarations.

9. SIGNATURES

By signing, I confirm I’ve read the above information, and that I understand:
1) Taking part in the Research and Content activities is my choice: I don’t have to if I don’t want to.
2) I’ve had a chance to ask questions if I want to and I can ask more at any time.
3) What I share in the conversation will be used to help Google understand what young people think about the way they use technology in their lives. Any quotes in the report from me won’t include my name or identify me.
4) I have read and I understand and agree to the Participant Rights and Data in Clause 7 above.
5) If anything makes me uncomfortable, I want to stop and take a break, or there’s a question I don’t want to answer, that’s not a problem. Raising concerns won’t cause trouble for the researcher, it won’t change my relationships with Google or Livity, and it won’t mean I’m judged in any way.
6) I agree that this Consent and Release Form supercedes and replaces any prior consent form signed by me or my parent/legal guardian in relation to the Project.
