Consent Form and Privacy Statement for Market Research

Research Overview

Thank you for agreeing to participate in this Research Project , which is a research study concerning (“Research”) conducted by Accenture on behalf of its client who requires anonymity until the end of the Research to ensure that the Research is not biased. Note we will share details of the client at the end of the research if you request.

The Research being carried out is to understand your views towards early campaign material. The materials shown are not finalized, and your views will inform their future development.

This Research will include the gathering and use of your personal data being collected solely for the purposes of undertaking the Research project. The Research Team may use your responses to generate AI video, text and/or audio output. This means that anonymised research responses may be used to help generate illustrative or synthetic examples (for example, summaries or representative videos) to support analysis and reporting.

The results and output (including a recording of the research session) may be passed on to the Client if this is required for the purpose of the Research.

Privacy Statement

Before we begin, we would like to provide an overview of what personal data we may be collecting from you, and what that personal data may be used for. This Privacy Statement and request for consent explains how and why Walnut part of Accenture (“Accenture”) processes your personal data , who has access to such personal data and how you can exercise your rights in relation to such personal data. Information on Accenture (and if relevant, its representative) can be found here. Any Accenture entity located outside the European Union will be represented by Accenture PLC.

What personal data does Accenture process?

Accenture will collect personal data about you to achieve the purposes set out in this privacy statement. Accenture is processing the following personal data: Age, Disability/Health status, Working Status, Ethnicity, Living Status, Name, Gender, Email address, Phone number.

Purpose and Lawful Processing

Accenture may collect, use, transfer, disclose and otherwise process your personal data in the context of managing its relationship with you and for the purposes set out in the Privacy Statement portion of this document. You understand that you will be voluntarily sharing your personal data. You will not be subject to adverse consequences if you do not provide personal data.

Accenture will not use your personal data for any purposes that are incompatible with the purposes listed in this privacy statement, unless it is required or authorized by law, authorized by you, or it is in your own vital interest (e.g. in case of a medical emergency) to do so.

Access and transfer of your personal data

Your personal data will be accessed by employees of Accenture who have a need to know the information for the purposes described above.

As a global organization with offices and operations throughout the world, your personal data may be transferred or be accessible internationally throughout by Accenture’s global business and between entities and affiliates. Any transfers of your personal data to others by Accenture offices (including transfers from within the European Economic Area (EEA) to outside the EEA) will be governed by Accenture's binding corporate rules (BCR; a copy of which can be found here (https://www.accenture.com/us-en/support/binding-corporate-rules).

When needed, Accenture may share your personal data with third parties, such as third party service providers and public authorities.

Any transfers of your personal data to third parties outside the EEA will be based on an adequacy decision or are governed by the standard contractual clauses.

How long does Accenture retain your personal data?

We will retain your personal data 6 months to fulfill the purposes outlined in this privacy statement, unless a longer retention period is required or permitted by law.

We maintain specific records management and retention policies and procedures, so that personal data are deleted after a reasonable time according to the following retention criteria:

· We retain your data as long as we have an ongoing relationship with you;

· We retain your data for as long as needed in order to comply with our legal and contractual obligations.

· We retain your personal data where such retention is advisable in light of Accenture’s legal position (such as in regard of applicable statutes of limitations, litigation, or regulatory investigations).

Your rights with respect to the processing of your personal data

You are entitled (in the circumstances and under the conditions, and subject to the exceptions, set out in applicable law) to:

· Request access to the personal data we process about you: this right entitles you to know whether we hold personal data about you and, if we do, to obtain information on and a copy of that personal data.

· Request a rectification of your personal data: this right entitles you to have your personal data be corrected if it is inaccurate or incomplete.

Object to the processing of your personal data: this right entitles you to request that Accenture no longer processes your personal data.

· Request the erasure of your personal data: this right entitles you to request the erasure of your personal data, including where such personal data would no longer be necessary to achieve the purposes.

· Request the restriction of the processing of your personal data: this right entitles you to request that Accenture only processes

· your personal data in limited circumstances, including with your consent.

· Request portability of your personal data: this right entitles you to receive a copy (in a structured, commonly used and machine-readable format) of personal data that you have provided to Accenture, or request Accenture to transmit such personal data to another data controller.

If at anytime you would like to withdraw consent, or have any questions about your data processing, or how to exercise your personal rights in relation to your data, please notify [email protected]. Please note withdrawing your consent will not affect Accenture’s right to process personal data obtained prior to the withdrawal of your consent, or its right to continue parts of the processing based on other legal bases than your consent. In simple terms, you have the right to see what data we hold about you, correct it, ask us to delete it, or tell us to stop using it.

If, despite our commitment and efforts to protect your personal data, you believe that your data privacy rights have been violated, we encourage and welcome individuals to come to Accenture first to seek resolution of any complaint. You have the right at all times to register a complaint directly with the relevant supervisory authority or to make a claim against Accenture with a competent court (either in the country where you live, the country where you work or the country where you deem that data privacy law has been infringed).

Confidentiality

Any information you will be shown during the Research is owned by the Client or, in some instances, Accenture. Any information, written or otherwise, relating to brand concepts and ideas is confidential information belonging to Accenture or our Client.

You therefore agree to keep any information confidential and not to disclose it or discuss it with anyone or make reference to it on any social media outside the Research. Upon request, you will destroy, erase, or return any confidential information you have received during the Research.

Please note that you will not be permitted to use your mobile phone, tablets, or any other recording device (unless for emergency reasons) whilst taking part in any of the Research except where required to complete research activities (such as uploading images or videos). This is in the interests of protecting the confidentiality of any Client products and other confidential information during the Research and preventing any deliberate or accidental disclosure anywhere, including on social media.

Acknowledgement and consent for data processing

I hereby provide my consent to Accenture, and the third parties described in this document (“Recipients”) to process my personal data in accordance with the above Privacy Statement.
