Consent Form 

Grail Insights Ltd. (Escalent) and the Study Sponsor in the UK are the data controllers for this research project. They are responsible for determining the purposes and means of processing personal data collected during this study.  For any queries regarding data protection, please refer to our privacy statement at https://escalent.co/privacy-policy/. 

 
By signing this form, I consent to Grail Insights Ltd. (Escalent) and the Study Sponsor collecting, storing, and processing my personal data solely for the purposes of conducting market research and product development related to this study.  My personal data will not be used for marketing purposes or shared with third parties outside the scope of this research. 

Under the UK GDPR, my personal data is processed based on my explicit consent (Article 6(1)(a)).  I acknowledge that my participation in this study is entirely voluntary.  I understand that I am not obligated to sign this consent form, and if I choose not to, Grail Insights Ltd. (Escalent) will be unable to interview me or collect and process my study-related information.  Furthermore, I retain the right to withdraw my consent at any time, without affecting the lawfulness of processing carried out before my withdrawal.

I understand that I have the following rights under UK GDPR:

  • The right to access, rectify, or erase my personal data.
  • The right to restrict or object to processing.
  • The right to data portability.
  • The right to withdraw my consent at any time. For details on how to withdraw consent, please refer to our privacy policy at https://escalent.co/privacy-policy/

 
My personal data will be retained for as long as necessary to fulfil the purposes outlined in this agreement, including ongoing product development. Data that is no longer required for these purposes will be securely deleted or anonymized, unless retention is required for legal compliance 

I understand that certain forms of the Study Information may include my first name or other personal information, and I give consent to their use by Escalent for the purposes of administering the study. 

I hereby assign to Grail Insights Ltd. (Escalent) all rights, title, and interest in any ideas, concepts, or insights I contribute during this study, without any expectation of compensation. I waive any moral rights in connection with such contributions and acknowledge that they may be used by Grail Insights Ltd. (Escalent) for any internal product development or research purposes. 

 
I agree to keep all information disclosed during the study confidential indefinitely, unless and until such information becomes public through no fault of my own. I understand that: 

  • Any breach of confidentiality may result in legal action. 
  • I will not disclose study-related discussions, findings, or information to any third party. 

 
If my personal data is transferred outside the UK/EU, it will be protected under appropriate safeguards such as Standard Contractual Clauses (SCCs) in compliance with GDPR requirements. 

I, affirmatively consent to Grail Insights Ltd. (Escalent) collecting and processing my personal data in accordance with the terms outlined above.
