RESEARCH CONSENT FORM

You are being invited to take part in research conducted by the researcher, CACI Limited (“CACI”, “we”, “us”, or “our”), which has its registered office in CACI House, Kensington Village, Avonmore Road, London, W14 8TS on behalf of its client (the “Client”).

What this research is about and what it will involve
The purpose of this research is to understand more about how users use the Client’s website(s), product(s) and/or service(s), as applicable, to assist in optimising users’ future experience.
If you decide to take part by signing the consent form below, you will be asked to participate in:

Where and how will the research be conducted
The research will be facilitated by CACI. The research sessions will be recorded for the research team to review them and may be observed by the Client.
Research location: Remote

During the research sessions we may take notes; ask you to share your screen using an online communication tool (such as Zoom, Microsoft Teams, Skype, or Google Meet), and record it; and record audio and video.
Some clips or quotations may be taken from the original recordings to illustrate findings when putting together research reports. The report will not be publicly published and is typically considered as confidential information of the Client. Your personal data will be anonymised in the report, unless your prior written consent was obtained.

Your rights

Your participation in this research is voluntary. You have the right to withdraw your consent at any time (see below), access your personal data, restrict processing, rectify and with some exceptions request to erase stored personal data relating to you.
If you want to make a request to exercise any of your rights listed above or if you have any concerns about how your personal data is being handled, please contact our data protection team at [email protected].
When you submit a request, please indicate your name, the name of this project, your reasons for making the request, if necessary, and other details you think will be useful for us to comply with your request.
Your personal data

Your personal data will be processed to manage your participation in the research in accordance with UK and European Union data protection law (as applicable), in the EEA, in line with Regulation (EU GDPR) 2016/679, and in line with the UK GDPR and Data Protection Act 2018. Your personal data including name and contact details, and original recordings are processed and stored securely, then deleted in line with the Privacy Policy set out below.
How to exercise your right to withdraw
As mentioned above, you can withdraw your consent to the processing of your personal data at any time and can raise any concerns or areas of concern during the session.
If you would like to withdraw your consent at any stage before or after the session, please notify our data protection team at [email protected], and we will remove all your personal data from our system. Your other rights listed above under the “Your rights” heading will not be affected if the right of withdrawal is exercised. When you withdraw your consent, we will not collect additional information related to you. We may also erase the personal data we already collected. This will happen only if its erasure does not render impossible or seriously impair the achievement of the objectives of the research project.
Compensation
Your recruiter or CACI, may or may not have agreed that you will receive compensation for participating in this research. Such compensation, whether you have received it or not, does not affect any of your rights listed above under the “Your rights” heading. You will only receive compensation as and when your participation in the research is complete.
