Respondent Release and Consent Agreement

I consent to Ipsos-Insights, LLC, its subsidiaries, parent companies, affiliates, nominees, licensees, their successors and assigns, and those acting with its authority (“Ipsos”), to transfer the photographs, film, tape, video or other recordings of my research participation (the “Recordings”) by or on behalf of Ipsos during the course of the Credit Card Membership research to our client (the “Sponsor”). The Recordings will be shown to the Sponsor exclusively in the context of the study, not shared publicly, and not for any commercial purpose. 

 

I, the undersigned, understand that: 

  1. Ipsos and the Sponsor will record, photograph, and all aspects of my research participation, including my responses to survey questions and my name, face, voice, likeness, and image (my “Personal Data”) contained within the Recordings as part of the study for the purposes of research only. 
  1. My Personal Data whether in original or modified form, will be shared with the Sponsor. 
  1. Ipsos will not share or further transfer the Recordings to any other party. 
  1. Ipsos and the Sponsor shall have no obligation to process the Recordings or to process my Personal Data in connection with my participation. 
  1. The Sponsor will keep the Recordings for archiving and possible quality control purposes, for up to months.  
  1. Ipsos and the Sponsor will share the Recordings with colleagues and subcontractors in the United States. The Recordings will be stored in these countries. 
  1. I have a right of privacy and can request access, corrections, deletions of my Personal Data which is held by any of the identified parties within this agreement. 
  1. I have a right of privacy and can request that Ipsos and the Sponsor stop processing my Personal Data at any time.   
  1. A copy of the research Privacy Policy has been provided to me and I may write to [email protected] for any privacy right request, including ’25-046776-01’ in the subject line or by calling toll free at (833-931-1521). 

 

By signing this agreement, I:  

  1. Represent that I am over the age of 18 years and that I am free to enter into this agreement. 
  1. Understand that I cannot request payment or royalties for the use of my Personal Data in conjunction with this research. 
  1. Indemnify and hold harmless Ipsos and its assigns from and against all costs, damages, and claims arising from any breach or alleged breach by me of any representation, warranty, or agreement hereunder.
