Fresh Egg user research consent form

This form confirms the terms on which you agree to allow Fresh Egg Ltd (Fresh Egg) and Sharps Bedrooms Ltd (covering Sharps, Fittingly and Clever Closet brands) to record you and use, for the purposes set out below, the recording of you taken by Fresh Egg.

What this study is about

This research is being conducted by Fresh Egg on behalf of Sharps Bedrooms Ltd. The purpose of this study is to gather feedback on storage requirements and how users find information online and to understand customers experiences with storage solution suppliers. Your participation in this study will help us to understand how people use websites so we can make recommendations on improvements.

Your participation in this study is voluntary

You can take a break at any time. Just tell the researcher if you need a break. You can leave at any time without giving a reason.

Information we want to collect

We will ask you to attend a Microsoft Teams call either via your mobile or laptop depending on your allocation by Roots, during which we will ask you to answer questions about your storage experiences. We will then ask you to share your screen whilst you undertake some tasks. We will ask questions along the way and take notes to record any comments and actions. Sessions will be recorded and viewed by the user testing moderator and colleagues but will not be made publicly available.

How we ensure your privacy

People on the research team may view the sessions remotely. Other people involved in the user research may view a recording of the session in the future. These recordings will be treated as confidential and will not be shared outside our company or Sharps Bedrooms Ltd. We may use external third-party processors when recording your session and when otherwise processing your data. We may also publish research reports with a wider distribution than our client that include your comments and actions but your data will be anonymous. This means your name and identity will not be linked in our research reports to anything you say or do.

Additional Information

Fresh Egg Ltd is the controller of the personal data provided by you in accordance with this form and the research we are carrying out. We are responsible for this personal data (including the recording).

We will:

1. ensure that we comply with the requirements of all legislation and regulatory requirements in force from time to time relating to the use of the personal data;

2. sometimes use external third party processors to process your data on our behalf; and

3. process the personal data in the recording and your contact details in accordance with our Privacy Policy, receipt of which you acknowledge by signing this form.

It may be that one of our external third parties or you are based outside of the EEA and therefore there may be processing of data which involves a transfer outside of the EEA. Whenever we transfer your personal data out of the EEA, we ensure a similar degree of protection is afforded to it by ensuring at least one of the following is in place: where the country has been deemed adequate by the European Commission, where we use specific EU approved contracts or where the third party has binding corporate rules in place.

Confidentiality

By consenting to this Agreement, you agree to protect and to hold confidential any Information provided, used, or arising in connection with this research and Sharps Bedrooms Ltd. You agree that you shall not use or disclose Fresh Egg Ltd or Sharps Bedrooms Ltd information unless such information becomes part of the public domain. You specifically agree to keep in absolute confidence and not to disclose or discuss with anyone, even family members:

- Any information about the project that you provide

- Any information regarding any Elements which you are commenting on, testing, have tested or will test, including the fact that an Element exists

Otherwise, we will delete your personal data in accordance with our data retention policy.
