I agree to participate in the market research project about TV and Digital advertising for the agreed incentive as described. I understand that my participation is voluntary, and that I can withdraw consent at any time. I confirm that my participation does not violate any existing agreement I hold or create a conflict of interest. The thoughts and opinions provided will be my own.

I acknowledge that the Data Controller is Amazon. Kantar UK Ltd are members of the Market Research Society and the International governing body ESOMAR. We operate within their strict codes of conduct. Kantar UK Ltd will never pass your details to a third party without your express permission.

Your participation in this survey will involve:

Online group lasting 90 mins for a financial incentive of £60 in the form of an online PayPal payment provided by the recruiter (Take Part in Research) directly.

This TV and digital advertising project will be hosted on the Sago platform where you will have a discussion with the Kantar moderator who will lead the conversation, there will also be 3 other people like you taking part. We will share your email address with Sago only for Sago to send you an invitation to use their platform, in line with their terms and conditions and applicable privacy policy, which they will provide to you.

When participating in this group as described above, I understand that I will:

participate in a 90 min online group (using a webcam), (my “Contribution”)

I agree that if I use my mobile phone or tablet to provide my Contribution, that the mobile provider may charge me, depending on the contract that I have. I confirm that I have asked the bill payer’s permission for my device to be used.

I consent that my Contribution, will include the following personal data:

Personal data, including (but not limited to) my name, email address, telephone number; and which will be used for the following purposes (please tick to confirm): To facilitate any incentive payment To facilitate the TV and digital advertising project activities described above To conduct quality control activities e.g. Validate identity

Your email address only, will be shared with the platform provider in order for them to send you an invitation which is covered by their t&c’s/privacy policy

My voice, opinions and image will be recorded, listened to and viewed (including in real-time), in extract or in full or in written transcript, by members of the Kantar UK Ltd project team and the client to support the research findings. Members of the Amazon marketing/operational teams will view the sessions. Recordings of sessions will not be shared outside of the Amazon marketing team. If anyone viewing from Amazon recognises any respondents they will immediately stop watching. The Amazon members watching sessions are Andrew Riungu and Darren Rix

My voice, opinions and image may be provided to the client in reports, presentations or other materials which will not to be released into the public domain to support the research/survey findings.

I acknowledge that, during the course of the project, a number of ideas and or materials may be presented to and or discussed with me which are private and confidential and not available to the public (‘Confidential Information’). I understand that it is important that this Confidential Information remains private and I agree to keep this information secret and not communicate it to anyone else without Kantar UK Ltd’s express permission. This includes not sharing with my family, friends, colleagues or others known to me and includes, but is not limited to, digital sharing on social media/networks, instant messaging etc.

I also understand that I do not have permission to use and or present the ideas and or materials presented to and or discussed with me and I agree not to do anything which may harm and or compromise those ideas and or materials and or Kantar UK Ltd’s rights in them.

My Contribution (including the Content) will be stored securely for a maximum of 12 months after which I understand my original Contribution will be safely destroyed by Kantar UK Ltd.

I agree to unconditionally release Kantar UK Ltd from and against any and all claims, liabilities, demands, actions, causes of actions, costs and expenses arising out of my participation or contribution in the TV and digital advertising research project.
