I, research participant, agree to participate in the research project for the agreed incentive as described. I understand that my participation is voluntary, and that I can withdraw consent at any time. I confirm that my participation does not violate any existing agreement I hold or create a conflict of interest. The thoughts and opinions provided will be my own.

I acknowledge that the Data Controller is Verian’s (Verian Group UK Ltd.’s) client, the Department for Energy Security and Net Zero (the “Client”). Verian is a member of the Market Research Society and the International governing body ESOMAR. Verian operates within their strict codes of conduct and will never pass my details to a third party without my express permission.

I consent that my Contribution, will include the following personal data:

(i) Personal data, including (but not limited to) my name, email address, telephone number; and which will be used for the following purposes

a) To facilitate any incentive payment.

b) To facilitate the research activities described above.

c) conduct quality control activities e.g. validate identity.

(ii) My voice, opinions and image will be recorded, listened to and viewed (including in real-time), in extract or in full or in written transcript, by members of the verian project team to support the research findings

(iii) I understand that I may exercise my rights under the UK GDPR such as my right to access, amend and/ or erase my Personal Data by contacting Verian (Verian Group UK Ltd) at [email protected]. You can check our privacy policy here: https://www.veriangroup.com/uk-surveys

I acknowledge that, during the course of the project, a number of ideas and or materials may be presented to and/ or discussed with me which are private and confidential and not available to the public (‘Confidential Information’). I understand that it is important that this Confidential Information remains private, and I agree to keep this information secret and not communicate it to anyone else without Verian’s (Verian Group UK Ltd.’s) express permission. This includes not sharing with my family, friends, colleagues or others known to me and includes, but is not limited to, digital sharing on social media/networks, instant messaging etc.

I also understand that I do not have permission to use and or present the ideas and or materials presented to and/ or discussed with me and I agree not to do anything which may harm and/ or compromise those ideas and/ or materials and/ or Verian’s (Verian Group UK Ltd) rights in them.

My Contribution (including the Content) will be stored securely for a maximum of 12 months after the project concludes, after which I understand my original Contribution will be safely destroyed by Verian.

I agree to unconditionally release Verian (Verian Group UK Ltd) from and against any and all claims, liabilities, demands, actions, causes of actions, costs and expenses arising out of my participation or contribution in the research project.
