Consent to video/audio recordings and observations

(hereinafter referred to as the ‘Data Subject’)

to RSB Insights and Analytics Inc, , 476, Wilson Street, Fairfield, Connecticut 06825,

(hereinafter referred to as the ‘Institute’)
and Google LLC , Mountain View, California, United States
(hereinafter referred to as the ‘End-User’

We, the institute, would like to take this opportunity to thank you for your participation in the
study on the topic of Google Pixel Support. Please carefully read through the following text
and grant your consent with a signature at the bottom.
In March and April 2026, the institute will conduct online interviews and record them as video
or audio files via Zoom. These recordings will solely be used for research purposes within the
scope of this study by the institute and the end-user. Any other use for other purposes (e.g.
advertising purposes) is strictly prohibited.
The end-user shall not learn your surname or address.
The video and/or audio recordings will only be transferred to the end-user for inspection
after prior signature of an ‘End-user/appointed institute declaration of commitment’. The
end-user is subject to the following obligations in connection to this declaration:
1. Recordings and observations must only be made in compliance with the ‘Directive for
recordings and observations in market and social research’, without any exceptions.
2. Recordings and observations must solely be made for research purposes and used for
the objective of the study.

3. Any attempt to de-anonymise data or recordings must be refrained from and
prevented. The end-user is not permitted to produce any image, video or audio
recordings or take any notes on specific individuals or notes that could be linked to
specific individuals.
4. Only individuals that sign the enclosed ‘Personal declaration of commitment to
ensure compliance with data protection requirements in accordance with the General
Data Protection Regulation (GDPR)’ and send it to the institute/studio conducting the
study in advance are permitted to make observations and use the recordings.
5. Observations and uses of recordings are only permitted in closed rooms to which only
individuals that have signed the aforementioned ‘Personal declaration of commitment
to ensure compliance with data protection requirements in accordance with the General Data Protection Regulation (GDPR)’ and sent it to the institute/studio
conducting the study in advance are granted access.
6. Please consult this declaration of consent to learn the recipients of recordings.
7. All recordings are deleted within 3 months of receipt.
The institute is the controller for the processing of video and audio recordings as per Art. 4(7)
GDPR.
The institute shall erase all data within 6 months after the end of the study.
You are entitled to the following rights against the institute with regard to your personal
data:
• Right of access (Art. 15 GDPR);
• Right to rectification (Art. 16 GDPR);
• Right to erasure (‘right to be forgotten’) (Art. 17 GDPR);
• Right to restriction of processing (Art. 18 GDPR);
• Right to data portability (Art. 20 GDPR);
• Right to object to processing (Art. 21 GDPR);

With my signature, I consent to the processing of the video and audio files for the purposes
of this study and the transmission thereof to the end-user for research purposes by the
institute and its processors (as per Art. 28 GDPR, such as technical service providers,
videographers, translators, cloud providers, etc.) in accordance with Art.6(1) clause 1a GDPR.
I acknowledge that I reserve the right to withdraw my consent at any time with future effect.
I am not required to specify the reasons behind the withdrawal of my consent and it will not
result in any negative consequences for me.
If I have signed a non-disclosure agreement, this shall not be affected by the withdrawal of
my consent and will remain in force.
