End Of Study Questionnaire

END OF STUDY QUESTIONNAIRE

To be completed ONCE BEFORE you drop off your product.

This questionnaire will sum up your opinions and compare both your test product and your normal Duck product:
Please note the questionnaire will take you approximately 20 minutes to complete so please allow yourself time to complete all questions before starting!

Please ensure you input your User ID correctly each time, this is your FOUR digit user ID (e.g 1001, 2001, 3001) which we have assigned. If this is not entered correctly then your questionnaire will not be counted

Please ensure you read each question and review the scales before answering as errors can not be changed after submission.

Please note in order to receive your incentive you must complete the following:
A MINIMUM of 2 usage entries per week - but please remember you can do more!
Completing questions with free-text in full sentences with as much detail as possible.
Complete the end of week questionnaire's as instructed
